New Patient Registration
If you wish to register as a patient and live within our practice boundary (see map opposite) please ask the receptionist for a new patient pack - you will need to complete the registration form, the Online Questionnaire for New Patients and provide the following:
- New address including the postcode
- Previous address including the postcode
- Previous doctor details i.e. name and address of doctor
- NHS Number
- Proof of address, i.e. utility bill, bank statement, driving licence, rent book
We will not be able to register you unless you are able to provide proof of address e.g. rent book, tenancy agreement, council tax bill (see above).
If you are unable to supply this information, we will be able to see you for any medical emergency as an overseas visitor and this will attract the relevant fees for this service.
If you are visiting the country on holiday, and your treatment is not urgent, you can be seen as a private patient.
You will be able to claim this fee back via your travel insurance. Please ask for a receipt.
